CCAC Hosting Strategic Accountability Sessions

The Erie St. Clair Community Care Access Centre is set to host its 2nd annual Strategic Planning Accountability Sessions across Windsor-Essex, Chatham-Kent and Sarnia-Lambton.

Accountability sessions were first held in September 2014 ahead of the development and approval of the ESC CCAC Strategic Plan 2014/15-2016/17. While the first sessions turned to health and community sector partners, patients and families, academics and policy makers to help identify key priorities for change, the upcoming sessions will discuss with these stakeholders the progress of the work being done in relation to this strategic plan.

“Participants will hear about the success, challenges and current state of these deliverables,” said Caen Suni, Senior Director, Strategy & Performance Management. “We will have a broad discussion to ensure our planned activities are still relevant or if we need to somehow change course in light of new information or trends. As always, we are keen to hear from our partners if there are other ways that we can work together to further our common objectives.”

The three Strategic Directions are: Patient and Family Centred Care, Reducing the Impact of Chronic Disease and Enhancing Care Coordination. The CCAC also has three Strategic Enablers: Value for Investment, Technology and Being an Employer of Choice. The sessions will involve table discussion between attendees and CCAC representatives based on these directions and enablers.

The Strategic Planning Accountability Sessions are scheduled for Chatham on Thursday, September 3rd, 2015 from 1pm to 4pm at the Erie St. Clair LHIN located at 180 Riverview Drive .

In Windsor it will be held on Tuesday, September 8th, 2015, from 1pm to 4pm at the Alzheimer Society of Windsor and Essex County located at 2135 Richmond Street.
